We send the submission documents in advance for you to make your monthly claim.
GP practices, pharmacies, appliance contractors, and MOD prescribing and dispensing sites will receive their submission documents at different times.

GP practices
FP34Ds and FP34PDs are printed on the 16th.
They'll be sent on the 17th of the month.
You can request a paper copy of your FP34 from the 25th of the month.
Pharmacies
You must submit your FP34C declaration on the Manage your service (MYS) portal by the fifth day of the following month in which the supply was made.
Each month you'll receive:
- a paper Account Identifier Document
- red separators
- pharmacy address labels
The Account Identifier Document must be sent alongside the paper prescription bundle. This will allow us to easily identify which pharmacy has submitted their prescription bundle which will help to speed up prescription processing.
If you lose the Account Identifier Document, you can download and print another copy from MYS.
Appliance contractors
FP34As are printed and sent on the eighth of the month.
You can request a paper copy of your FP34 from the 25th of the month.
MOD dispensing and prescribing sites
MO1 submission documents are sent on the 18th of the month.
You should expect to receive your submission document by the 25th of every month.
