An FP10CDF is a Controlled Drug Requisition Form. It must be sent at the end of the month along with the FP34PCD.

The FP10CDF and FP34PCD forms must be submitted to Prescription Services processing division in Newcastle. This is regardless of where you send your normal monthly submission.

If the requisition is not received on an FP10CDF, it will not be accepted.

Using the FP10CDF

All requisitions used for the supply of schedule 1, 2 and 3 CDs by authorised practitioners working in any health care setting in the community, for the purpose of treating patients, must be declared using the form FP10CDF.

Community pharmacies must use their private OCS code as the NHS account number on the FP10CDF. 

If you do not have this code, contact your Integrated Care Board (ICB). The ICB will set up the code.

Dispensing doctors should not supply controlled drugs ordered on a requisition unless they have a wholesaler's license, as normally they're not permitted to carry out a wholesale function.
