If the FP10 is computer generated and your age or date of birth is clearly visible, a signature is not required and you do not need to tick the box on the back of the prescription as you are classed as automatically age exempt.

If your age on the front of the prescription says 59, but you are getting the prescription dispensed after you have turned 60, then you will need to sign and tick the exemption on the back.

If the prescription is hand written, the FP10 must be signed and the relevant box on the back of the prescription should be ticked.
