Do patients have to sign and complete the declaration on the back of the white EPS tokens?

Yes, even though the information is transmitted electronically for processing, it is still a requirement for a patient to make an exemption declaration. 

The only exception to this would be for patients who are age exempt with their date of birth contained within the electronic message.

These declarations can be collected either on a dispensing token printed by the pharmacy or on the prescribing token given by the prescriber.
