How to apply
Contact NHS Dental Services to request a D7 or D7A form.
We're unable to consider a request if:
- the patient has been seen by a Clinical Advisor within the last 12 months for the same issue
- the request is for advice about a claim for damages or compensation under civil litigation
Once a patient has been examined by a Clinical Advisor, a report will be produced with their findings within 2 weeks of the examination.
