If a patient has moved to your area and wants to continue an active course of orthodontic treatment with your practice, you need to open a new course of treatment (CoT). As this would be the patients second CoT, you must contact your Commissioner (Sustainability Transformation Partnership / Local Health Board) for confirmation they would allow this.

If the Commissioner agrees to this, you will be able to submit a new FP17O once the original CoT has been closed by the previous practice.

In these cases, the patient's original IOTN and age at the start of their first CoT applies. Contact the previous orthodontic practice to get this information.
