Opting out

To opt out of the NHS Pension you must complete a SD502 application form

You can download this form and view the guidance for its completion on the NHS Pensions website.

Once the SD502 form has been completed by both the performer and NHS Dental Services, the form is sent to NHS Pensions and NHS Dental Services will keep a copy. This form can only be sent to us by post.


Claiming a refund of contributions

You must complete ‘Part A’ on the RF12 application form and have less than 2 years membership to claim back a refund of your contributions.

This form can only be sent to us by post, we will log the application within 48 hours of receipt.

NHS Dental Services will complete employer ‘Part B’. Once fully completed, the RF12 form is then forwarded to NHS Pensions to process the refund.

If you have been paying contributions in error, then this process will not apply to you, and you would need to contact your Commissioner (Sustainability and Transformation Partnership / Local Health Board) to enquire about any refunds. This is referred to as a local refund.
