Your dental practice will charge you if you need a replacement for a lost or broken: 

  • NHS denture, bridge or orthodontic appliance in England 
  • orthodontic appliance in Wales 

This is sometimes referred to as a ‘Regulation 11 replacement appliance’.

The current charge is:

  • £99.60 per appliance in England
  • £86.40 per orthodontic appliance in Wales

You must pay the NHS charge even if you normally qualify for free or reduced cost NHS dental services.

You may be able to claim a refund if:

  • paying the charge caused you undue financial hardship
  • you took reasonable care of the appliance
  • you were receiving a qualifying benefit or named on a valid exemption certificate at the time of treatment

To claim a refund, download and print an FP17R11 form and send it with your receipts to:

You must complete part 2 with information about the appliance's loss or damage and submit the original receipt. We cannot accept a photocopy. 

If you've lost your original receipt, your dental practice can provide you with a duplicate.
