If you're entitled to a medical exemption certificate, your doctor or a health professional who has access to your medical records will complete an FP92A application form.

The FP92A must be an original document. We cannot accept a:
- photocopy
- email attachment
- downloaded copy
- printed copy

Practitioners must use the Primary Care Support England (PCSE) portal.
Integrated care boards (ICBs) and Sub ICB Location (SICBL) must use Xerox UK limited. Your organisation must be registered to use this service.
