If your doctor, midwife, or health visitor cannot apply online on your behalf for a maternity exemption certificate, they must complete an FW8 application form.
This must be signed by you and the doctor, midwife, or health visitor completing the form.

If you live in:
- Scotland, post this to the regional department within the Practitioner Services Division of NHS National Services Scotland
- Wales, post this to your Local Health Board (LHB)
- Northern Ireland, the HC11A application form is sent to the Business Services Organisation (HSC)
The FW8 must be an original document. We cannot accept a:
- photocopy
- email attachment
- downloaded copy
- printed copy

Integrated care boards (ICBs), Sub ICB Location (SICBL) and Local Authorities must use Xerox UK limited. Your organisation must be registered to use this service.
