An FW8 is the application form for a maternity exemption certificate.

This form can only be completed by a GP, midwife or medical representative who has access to your medical records.

The FW8 must be signed by both the applicant and their GP, midwife or health visitor completing the form.

When sending a paper FW8 form, this must be an original document and not a photocopy or email attachment.

Primary care practitioners can order FW8 application forms and RE26 return envelopes on the Primary Care Support England (PCSE) portal.

Integrated Care Board (ICB), Sub ICB Location (SICBL) and Local Authorities can order FW8 application forms and the RE26 return envelopes from Xerox (UK) Ltd. Your organisation must be registered to use this service.
