If original documents are requested, we cannot accept copies unless the copy has been certified by a third-party in a professional capacity.

We must be able to identify the third-party.

We cannot accept copies if the person certifying the document is either:

  • related to the applicant
  • in a relationship with the applicant
  • lives at the same address

To certify the copy, they must:

  • see the original
  • write 'Certified to be a true copy of the original seen by me'
  • sign and date it
  • print their name under the signature
  • add their occupation, address, and telephone number

Lasting Power of Attorney (LPOA)

For LPOAs, the donor can certify a copy if they’re able to make their own decisions.
