An HC1(SC) must be completed if you either:
- live permanently in a care home, the local authority helps with the cost of your care and your savings are less than £23,250 or £24,000 if you're a resident in Wales
- have recently left local authority care, you're supported by a local authority and your savings are less than £16,000 or £24,000 if you're a resident in Wales
If you're a resident in a care home, the local authority does not have to cover the full cost of your care. If they only pay part of the fees, you can complete an HC1(SC) form.
England

Wales
You can request an HC1(SC) from the Welsh Government publications line 0345 603 1108. You must select the option for health publications.
Scotland
If you are resident in Scotland, you can collect an HC1(SC) from your local pharmacy.
