Your personal details on your PPC must match your GP records to avoid a Penalty Charge Notice (PCN).

If you’ve changed your address, you must tell your GP before contacting us.

You'll not receive a replacement certificate as your current PPC will remain valid.
Name
If your PPC shows the wrong name or you’ve changed your name, you must send us evidence of the correct name.
We’ll accept:
Wrong name
- birth certificate
- passport
- driving licence
- marriage certificate
Changed name
- passport
- driving licence
- marriage certificate
- civil partnership certificate
- deed poll including un-enrolled
If you have a paper certificate, you must destroy it.
If you have any prescriptions to collect whilst waiting for your new PPC to arrive, tick ‘box F’ on the back of your prescription.
Once we receive your evidence, you'll be sent a digital certificate by email within 24 hours.
If you have a paper certificate, your new certificate will be sent by post within 10 working days.
Date of birth
If your PPC shows the wrong date of birth, you must send us evidence of the correct date of birth.

We recommend sending photocopies. If you send us original documents, these will be returned within 20 working days from the date you sent them.
If you need a PPC once your current certificate expires, it's your responsibility to make sure you purchase a new PPC if your certificate does not automatically renew.
