If you need to update your name or date of birth, you must send us evidence by email.

Name
If your HRT PPC shows the wrong name or you’ve changed your name, you must send us evidence of the correct name.
Wrong name
To update a name that is wrong on your certificate, send us a copy of your:
- birth certificate
- passport
- driving licence
- marriage certificate
Changed name
If your name has changed since you got your certificate, send us a copy of your:
- passport
- driving licence
- marriage certificate
- civil partnership certificate
- deed poll including un-enrolled
If you need to collect a prescription before the HRT PPC arrives, you can still collect your prescriptions by ticking ‘box W’ on the prescription form.
Not all prescription forms have a tick box for HRT PPCs. If that's the case, you must tick box 'F' for Prescription Prepayment Certificate (PPC).
Date of birth
If your HRT PPC shows the wrong date of birth, you must send us evidence of the correct date of birth.
