DCP has completed the full CoT

If the DCP has completed the full CoT without a dental performer, they must submit their own FP17.

These are submitted using the DCPs Personal ID.

DCP has completed part of a CoT

If DCPs share a CoT with a dental performer, the dental performer’s Personal ID must be included on the FP17.

When completing an FP17 you must include:

  • the type of DCP (such as a dental therapist or a dental hygienist)
  • the DCP’s GDC number, this is validated against the GDC register on Compass

Both fields must be completed. If you only complete one of the fields, the FP17 will reject.
