Name
If your name is incorrect or you've changed your name, you must send us evidence.
If you're receiving Universal Credit, you must update your name with them.
Incorrect name
You must send us either your:
- birth certificate
- passport
- driving licence
- marriage certificate
Name changed
You must send us either your:
- marriage certificate
- civil partnership certificate
- deed poll

We'll post a new NHS Healthy Start prepaid card to you within 10 working days of receiving your evidence.
When you receive your new card, you must destroy the old card.
