If you've received an enquiry letter, our checks were unable to confirm you held a qualifying benefit:

  • at the time of your dental treatment
  • when your prescription was collected

Your details on your qualifying benefit must match the details with your dental practice or GP surgery.

If your details do not match, you can confirm them online. If you do not have access to the internet, contact our helpdesk.

If a match is found, your case will be closed.

If a match is not found, you must send evidence of your qualifying benefit by email or post.

Evidence

To confirm your qualifying benefit, send us a copy of your benefit award letter which covers the time of your treatment or prescription collection date. We must see all pages of your award letter.

You can get a proof of benefit letter from the Department for Work and Pensions (DWP).

For Universal Credit (UC), your evidence must show your total take-home pay on an assessment period which covers your time of treatment or prescription collection date.

To access your UC statements online:

  1. Login to your UC account.
  2. Go to the ‘Statement Summary’ page.
  3. Select the relevant statement - the statement will appear on your screen.
  4. Select the option to ‘Print to PDF’ or ‘Save as PDF’.

Before sending in evidence, you can check the ‘Other support you may be able to get’ section of your statement. This will tell you your total take-home pay and show you any additional support you may be entitled to.

If your name or date of birth does not match our records, you must also send evidence to confirm your correct details:

If you’re sending evidence by post, you must send a photocopy of the original document. If you send an original document, we’ll return this with your response.
