If you paid for NHS treatment before finding out you were entitled, you may be able to claim a refund.
Refunds must be claimed within 3 months of the date you made payment.
If you live in England
You can apply for a refund of NHS dental charges by downloading and completing a HC5(D) form.
You can send the form to us by email or post, unless you receive a War Pension payment or an Armed Forces Compensation Scheme payment.
If you receive one of these payments, you must submit the form by post to the Service Personnel and Veterans Agency. The address is shown on the HC5(D) form.
If you do not receive a War Pension payment or an Armed Forces Compensation Scheme payment, send the completed form to:

We cannot accept proof of payment from:
- credit card receipts
- bank statements
- treatment plans
It takes up to 8 weeks from the date your application is received for your refund to be paid.
Refunds for costs in England will be paid by Bacs (Bankers Automated Clearing System) transfer. You must give your bank account details on your HC5. Your application will be returned if this is missing.

If you live in Scotland
To apply, download, print and return an HC5 refund form with your original receipt. The HC5 form tells you where to send it.

Visit the Wales help with health costs website to download leaflets and forms.
You can also call 0345 603 1108 and select the option for health publications to get a copy sent to you.
